For Patients
To enable The Bondi Dentists to provide you with the highest level of care, your first appointment
is the most important.
At or before your first visit, you will be asked to complete a medical and dental health
questionnaire. This information is held in the strictest of confidence and will be used to assist us
in diagnosis and treatment planning, and will help to ensure your safety.
You can, if you wish, download this questionnaire from this website and fill it out in the comfort of
your own home and bring it with you to your appointment.

A comprehensive dental exam (including diagnostic x-rays, if required) will be performed by your
dentist at your initial dental visit. Your dentists will take the time to understand your needs and
desired results and will listen to, and answer, any concerns you may have relating to any aspect of
dentistry, our practice, or your treatment.
EXISTING PATIENTS
It’s important to get your teeth checked at regular intervals, at least once per year. Your dentist
and hygienist will give you a comprehensive examination (including diagnostic x-rays, if required)
to help in the early detection of decay or any other problems. We will also check your gums
for any signs of periodontal disease, check your teeth for decay and also check your existing
restorations such as fillings, crowns etc.
If it’s been a while since you had a check-up, contact our friendly team to arrange a consultation.
Your appointment and payment policies
Appointment alterations – Please give us at least 24 hours notice if you need to reschedule your
appointment. Late notification or failure to attend an appointment may incur a fee.
PAYMENT
Payments are expected on the day of your appointment. However, multi stage treatments are
subject
to a deposit at the first visit and settlement of the full account at the last appointment.
We accept payment by cash, cheque, eftpos and all most major credit cards.
Dental Insurance Companies (commonly known as “Health Funds”)
We are pleased to offer HICAPS at our office, which is an electronic facility to help patients make
their health insurance claims on the spot: at the office on the day of their procedure through a system
that uses their health insurance card. Most major health insurance companies are accommodated.
For your convenience we are happy to provide you with appropriately coded accounts, receipts and
quotations to enable you to deal with your insurer as efficiently as possible. The rebates you receive
from your private health insurance company is determined solely and agreed to by yourself and the
company: we are not able to have any influence or affect this in anyway.
Infection control
Your safety and protection, care and comfort are of the highest priority. We have therefore implemented
stringent infection control procedures within our practice and these are followed meticulously by
every team member. Our practice uses the most advanced sterilisation techniques and disinfection protocols.
Each member of our team is highly trained to follow a detailed process when it comes to the cleaning
of each surgery. Your protection begins long before your appointment and continues during every phase
of your treatment. Our dental team are trained with the most current infection control regulations as
specified by the Australian and New Zealand Standards (AS/NZS 4815), the Dental Board of Australia,
the Australian Dental Association and the National Health and Medical Research Council.
